SCHEDULE APPOINTMENTS

To schedule an appointment, please call (212) 935-9300. Same day or next day appointment requests will be given, based upon availability. New patients are always welcome.

 

Mon - Thr: (8:00am - 5:00pm)

Friday: (8:00am - 2:00pm)

CANCELLATIONS

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient. Patients will be billed for late cancellations or no-shows. Please schedule only definite appointments. There are no tentative appointments.

Payments, co-payments and co-insurances are due at the time services are rendered. Forms of payment accepted by the office are check, cash, insurance assignment and the following credit cards:      

 

 

 

For extensive treatment, payment plans may be arranged with the office manager. Patients may review a treatment authorization form and financial agreement before treatment is initiated. For crowns, bridges and other major work, full payment must be made prior to completion of the treatment.

EMERGENCIES

Emergency Care and After-Hours Support At Madison Dental Group, we understand that dental concerns can arise unexpectedly. While our on-call service is available 24/7 to answer your questions and provide guidance, please note that after-hours in-person care is referred to our trusted specialists. If you require immediate attention outside of our operating hours, please contact one of the following:

If you are experiencing a true dental emergency, such as significant trauma, uncontrollable bleeding, or severe pain, please visit your nearest hospital for immediate assistance. We’re here to guide you through any dental concern and ensure you receive the care you need, when you need it.

TELEPHONE CALLS TO THE DENTISTS

If you have medical questions which only one of our dentists can answer, a message will be taken and your medical file will be given to a dentist who will return your call the same day. If your question is non-medical, a qualified staff member will be pleased to assist you.

MEDICAL RECORDS

All medical information is kept strictly confidential. Our records cannot be released without a written request. We are HIPAA compliant.
